Lost Authenticator

If you’ve lost access to the device used for two-factor authentication (2FA), you'll need to verify your identity before you can reset your 2FA settings.

This process cannot be completed on a mobile device. You will need access to a desktop or laptop computer with a working webcam.

1. Go to https://app.tenet.ky/auth/signin and enter the username

2. Click on the "NEXT" button

3. For your browser settings, click "Allow" for know your location

4. Enter the password

5. Click the "SIGN IN" button

6. Click on "Unable to submit one-time code?"

7. Click on "I have lost my authenticator"

8. Click on "START" button

9. Enter a mobile phone number that can receive one-time codes via text messaging

10. Click on the "NEXT" button

11. Upload a copy of the user’s "Passport" in an acceptable format (JPG, PNG, OR PDF)

12. Click on the "NEXT" button

13. To confirm the identity of the user, click "I am ready" to take a photo

14. Click the “Take photo” button

15. When satisfied when the photo, click the "Save" button to submit

16. A confirmation screen will appear. If you have not received a notification after 72 hours, please contact Client.Services@tenet.ky
